Case Manager
Position Overview
In this role, you will provide Medical and Non-Medical Case Management, using proactive case management to serve Clients who have multiple complex psychosocial and/or health-related needs, focusing on maintaining them in systems of primary medical care to improve their HIV-related health outcomes. You will also provide access to area support services to meet non-medical needs.
